I am sure you are aware of the many benefits, among which is life insurance, that accompanies membership in the State Association. It is time again to take care of your dues so you will be covered for the 2007-2008 school year. Please review the enclosed brochure containing pertinent information about membership in the State Association.
Kentucky State dues are $50.00 a year, which includes life insurance if received before September 30th. If dues are received after September 30th, the life insurance will not be included. With your paid 2007-2008 membership you will receive: $5,000.00 life insurance policy, membership card, and new members will receive the above plus KHSADA lapel pin and foil membership certificate.
You will receive in a separate mailing from the NIAAA your renewal for NIAAA membership for 2007-2008 You must return your NIAAA membership dues and enrollment form directly to the NIAAA. DO NOT RETURN NIAAA DUES/FORM TO THE KHSADA.
Enclosed please find the KHSADA membership brochure. Please complete the enrollment form,
the requested information on this letter and make a copy of both documents and
return the originals of each form to the address listed below.
